REGULAR CHECKUPS

  • Download the CDC’s Milestone Tracker App
  • We encourage parents to follow the American Academy of Pediatrics recommendations for well child checkups which are as follows:
  • First year- hospital follow up visit (usually 2-3 days after discharge), 2 weeks, 1 month, 2 months, 4 months, 6 months, 9 months and 1 year.
  • Second year- Visits at 15months, 18 months and 2 years of age.
  • Preschool/School aged children- schedule visits annually.
  • Note that most insurance requires 365 days to pass between check up visits. 
  • An exception is Connecticut Medicaid (Husky Health) which allows 1 check up every calendar year.
  • For children with chronic medical problems (for example: asthma, eczema, seizures, ADD/ADHD) or on chronic medications you will be seen more frequently for follow up.
  • A day before your appointment you should receive an email regarding the Ages & Stages Questionnaire, a developmental screening tool, to be completed prior to your appointment.
  • If requested, you will receive a CT State Health Assessment form which can generally be used for school, camps, sports, etc.  If you have specific forms that must be completed please bring at the time of visit.
  • These visits are a chance to learn as much as you can about the best ways to help your child grow and thrive! 
  • We do prefer you come to your child's appointment, at least one parent, until your child turns 18 years of age. However, once your child is able to drive themselves, you and/or they may wish for them to come alone to some appointments. If this is the case, we require written permission to see the child without a parent/guardian. This can be emailed, faxed or sent to us via the patient portal PRIOR to the appointment.
