Note that most insurance requires 365 days to pass between check up visits.
An exception is Connecticut Medicaid (Husky Health) which allows 1 check up every calendar year.
For children with chronic medical problems (for example: asthma, eczema, seizures, ADD/ADHD) or on chronic medications you will be seen more frequently for follow up.
A day before your appointment you should receive an email regarding the Ages & Stages Questionnaire, a developmental screening tool, to be completed prior to your appointment.
If requested, you will receive a CT State Health Assessment form which can generally be used for school, camps, sports, etc. If you have specific forms that must be completed please bring at the time of visit.
These visits are a chance to learn as much as you can about the best ways to help your child grow and thrive!
We do prefer you come to your child's appointment, at least one parent, until your child turns 18 years of age. However, once your child is able to drive themselves, you and/or they may wish for them to come alone to some appointments. If this is the case, we require written permission to see the child without a parent/guardian. This can be emailed, faxed or sent to us via the patient portal PRIOR to the appointment.
